Everything You Need To Know About Admission

Moving into a nursing home can be a stressful time so we will always do our best to help make the transition as smooth as possible.

Usual Key Steps Involved In Moving Into One Of Our Nursing Homes:

1. Private Discussion With Our Care Coordinators

We are here to listen, and offer our help. During our private discussion, our Care Coordinators will advise you on the care services suited to your needs, guide you through the admission process, and provide you the estimated care charges depending on the level of care required and your choice of room type.

The following documents are required* prior to admission, for our nursing and medical team to assess admission eligibility and to institute an appropriate care plan according to your medical needs. 

  1. Recent medical documents (e.g. discharge summary/ in-patient summary from hospital, or memo from General Practitioner)
  2. Chest X-Ray Report
  3. Medicine List (if applicable)

All medical documents should be dated no earlier than six (6) months from the intended date of admission.

Prior to admission, our Care Coordinators will provide  a (face to face)  financial counselling session with you to assist you through the admission documents. 

A copy of List Of Things To Bring will be shared to prepare you when moving in.

Dedicated medical transport services will be provided for a fuss-free admission. 

Our Nursing Home is able to accept residents receiving subsidy from the Ministry Of Health. Subsidies are subject to means testing by the Agency Of Integrated Care. Please contact us for more information on subsidy eligibility.
