At our nursing home, we focus on delivering person-centred dementia care and improving the ageing experience for residents through a multi-disciplinary approach comprising various stakeholders. Everything begins with our integrated team of healthcare professionals, who will holistically evaluate the needs and care planning for each resident and work in tandem with residents and their next-of-kin (NOK) to render optimal care.

Alongside providing quality dementia care, we understand the need to let our residents stay socially active and engaged, promoting active ageing by keeping their minds busy and helping them stay healthy and positive. That is at the core of our dementia care programme – dubbed HOME (Healing Of Mind through Engagement) – which encourages residents to participate in small group cognitive activities, such as cooking, pet therapy, reminiscence therapy, music therapy, and outings.

Through this tailored dementia care programme, residents are encouraged to move away from the sense of solitude and helplessness that some may experience when residing in an institutionalised care setting. Our team of medical professionals will perform an initial assessment of the senior’s complex needs. Based on the findings, our occupational therapist or dementia practitioner will develop a customised individual care plan suitable for the patient’s abilities and strengths.

In addition, patients will be given choice and autonomy in deciding the programmes they would like to participate in. From stretching and strengthening exercises to sensory and grooming activities, they can choose from a broad range of activities that suit their interests and capabilities. 

Caregivers are also not neglected as part of our programme, as we understand that caring for someone with dementia can be stressful.
