What to Prepare Before Admitting a Loved One to a Nursing Home in Singapore

Pre-Admission Checklist

1. Medical Documents

Before admission, most nursing homes in Singapore, including Orange Valley, require a set of medical reports to assess the resident’s care needs.

Ensure you have:

  • Latest chest X-ray report (within the last 6 months)
  • Doctor’s memo or hospital discharge summary stating medical conditions (dated within 6 months)
  • Up-to-date medication prescription list
  • Vaccination records – Resident must have completed two Covid-19 primary doses and at least one booster dose.
    • Note: Exemptions will be reviewed on a case-by-case basis during the initial assessment.

2. Financial Documents

If you’re exploring MOH means-tested subsidies, here are important steps to take before submitting your application:

  1. Approach a Medical Social Worker (MSW) at any restructured hospital or community hospital for assistance.

  2. Alternatively, visit www.aic.sg or call 1800 650 6060 for support.

  3. If the applicant has had no prior contact with public healthcare institutions, reach out to your Community Development Council (CDC) for further guidance.

Be sure to inform the MSW that Orange Valley is your preferred nursing home, so your application can be processed with this preference in mind.

3. Application Forms

Before admission into Orange Valley Nursing Home, you’ll be asked to complete several standard forms. These help us ensure a smooth, transparent, and well-coordinated care experience:

  • Resident Admission Application
    • Collects essential information to begin the admission process.

  • Transport and Movement Consent

    • Seeks approval for the arrangement of transport (e.g. ambulance) for medical appointments or emergencies when needed.

  • Terms and Conditions of Care

    • Outlines the general care guidelines, service terms, and responsibilities of both the nursing home and family members.

  • Financial Acknowledgement

    • Provides a summary of estimated charges and payment options, including subsidy arrangements if applicable.

4. Personal Items for Comfort

  • Spectacles, dentures, or hearing aids if needed

Avoid bringing valuables or breakables belongings.

5. Emotional Preparation

  • Scheduling a pre-admission visit to the facility

  • Involving your loved one in conversations

  • Meeting the care team early for reassurance

  • Preparing family members, especially children, about the change
